A grind on the front peg with the back part of the bike brought over the railing.
How do it
After you've learned how to do a toothpick, it's time to move on to the toothpick hangover. It's best to practice it on low railings. What you do is grind on the front peg and bring the rear of the bike over the railing.
1.
You can prepare for this trick by doing a nose manual over the rail, going sideways/diagonally. This will help you get a feel for bringing your rear wheel over to the other side.

Enter a toothpick closer to the end of the rail, shifting your body weight forward and sideways over the rail. Have your back leg lightly push the bike frame to the side, so that your rear wheel goes over the rail. At the end of the rail, do a low nollie, pull the handlebars towards you and go down on both wheels. It's a rather demanding trick, so don't forget to oil the railing for an easier grind.
